Patient Registration
When you register at the practice you will be asked to complete a registration form and also a questionnaire, this is because it can take a considerable amount of time to receive your records. We MUST receive your registration form and fully completed questionnaire, along with your proofs of identity before we can register you.
You must bring along one proof of identity and one document that confirms residency from the lists below. Pease note we cannot register you without these being provided
Documents to confirm identity:
- Birth Certificate (Not a copy)
- Passport
- UK photocard driving licence
- Benefit / Pension Book (Not card)
- Official Tax notification
Documents to confirm residency:
- Housing contract
- Utility Bill dated within the last three months (not mobile phone)
- Bank statement
- Residence Permit
- Letter from IND or Border & Immigration Services
- Official tax document (eg council tax)
- Work Permit
If this is your first registration with a GP in this country, you will also need to provide a current visa when you register. Please note that checks will be carried out with both the Home Office and the Fraud Department to ensure that you are entitled to register as an NHS patient.

Core Areas:
Please note that each practice has a core area to accept registrations from - we DO NOT accept registrations from Southcourt, Mandeville, Fairford Leys, Meadowcroft, Hayden Hill, Aston Clinton, Stone or Bishopstone. If you are registered and move out of the area, then you will be required to re-register at a practice, which is more local to you. If in doubt please consult reception.
